• Each form must be COMPLETED by hand
• Physical signature is REQUIRED wherever asked. Electronic signatures will not be accepted for these purposes.

Step 2

Complete the following form and upload required application forms from Step One.

Step 3

Thank you for submitting your application.

This step happens in office at ACI. We will:

1. Review your application for completion and requirements of service compliance / agreement
2. Request written references from the reference contacts you provided on your application form
3. Submit your final reviewed application to our Board of Directors for final approval

This step can take up to 2 weeks for completion. Once this step is complete, our office will email you with our decision and proceed to Step 4.

Step 4

Final Approval & Account Setup

Once your application is approved, we will email you several forms to complete, sign and provide us with all administrative information necessary for our database, your ministry file and for tax purposes.  Once you complete and return these forms, you are welcome to begin utilizing ACI to receive and process gifts for your ministry.
